Kindly please go through the list if the certificate are still not collected.
 
The certificates can be collected at our office Medical Device Authority, Ministry of Health, Level 6, Prima 9, Prima Avenue II, Block 3547, Persiaran APEC, 63000 Cyberjaya, Selangor Darul Ehsan. 
 
Collection hour   :  8:30 am - 4:00 pm
Break  :  1:00pm - 2:00 pm (Monday to Thursday)
   :  12:15 pm to 2:45 pm (Friday)
REMINDER
Kindly bring your authorization letter from owner of the establishment and print the list and highlight your form ID number(s) for acknowledgement purpose during your certificate collection.
IMPORTANT NOTE
Uncollected certificate within 30 days from the date of this announcement will be disposed automatically without affecting your right to make a fresh application again.
